Criminal History Information Request

Clinical facilities that the University of Oklahoma College of Allied Health uses for clinical experiences (other than the OU Medical Center) are requiring Oklahoma State Bureau of Investigation (OSBI) background checks, including a check of the Sex Offenders list.

In preparation for clinical you will need to have an OSBI background check completed. If you are not required to obtain the OUMC background check, you may not need an OSBI check. Confirm this with your clinical coordinator.

To obtain a copy of the criminal history record you will need to fill out the Criminal History Information Request form. Requests can be made in person Monday through Friday (except state government holidays) between the hours of 8:00 am and 5:00 pm.

The fee can be paid in cash, by money order, Visa, Mastercard, Discover, cashier's check, certified check made payable to the Oklahoma State Bureau of Investigation (OSBI). No personal checks will be accepted.

In order to do this, you will need to:

  1. Go to Oklahoma State Bureau of Investigation
    Criminal History Reporting Unit
    6600 North Harvey, Building Six, Suite #140
    Oklahoma City, OK 73116 (Map to OSBI)

  2. Request an OSBI background check including a check of the Sex Offenders list.

  3. Pay $15.00.

The check will take you approximately 15 minutes to do. Results will be given to you that same day.

Keep your background check in a safe place that you will remember. You will need to present the background check to facility personnel the week prior to your first clinical experience.
