    To apply to the Web-Based Doctor of Science Degree Program, complete the following steps and mail the information (along with check or credit card information) in a packet to:

    Oklahoma City, OK 73190

    1. Complete the online application to the University of Oklahoma Health Sciences Center.
    (*Major code for the Web-Based Doctor of Science Degree Program is 1220D.) Print a copy of the application to include in the packet.

    2. Pay non-refundable OUHSC application and processing fees. Students may send one check payable to OUHSC or use a credit card and mail to Marsha Collier (along with packet).

    OUHSC Application Fee .....................................$40.00
    OUHSC College of Allied HealthProcessing Fee........$25.00

    3. Send official, sealed transcripts from all schools attended. If you graduated from the University of Oklahoma and have not taken courses since then, you do not need to submit transcripts.

    4. Submit a career goal statement.

    5. Submit three letters of reference from individuals who can describe your potential for success in a doctoral level program.

    6. Take the Graduate Record Exam (GRE). Scores are due by the application deadline. Applicants are strongly encouraged to take the GRE at least two months prior to the admission deadline.

    Applicants who previously have taken the GRE and graduated from a professional or postprofessional program that required the GRE are not required to take it again. Please complete and submit the GRE waiver form.
